Care at Home

Care at Home is a medical practice that provides virtual and in-home care designed to supplement and support your primary care.

Care at Home is another way CINQCARE delivers health and well-being, where you live. Physicians, nurse practitioners and physician assistants assess, diagnose and provide treatment via video conference and/or in-person home visits as determined by your choice. These providers work hand-in-hand with your family and caregiver to render labs, tests and procedures in your home when needed, which saves you travel and waiting time. Depending on your needs, you will be directed to either the Care at Home Annual Assessment Visit Program or the Care at Home Concierge Program. Select a program below to learn more.

CinQConnections

Care at Home is one of many ways CINQCARE provides quality connections for the Family Members it serves. It’s part of what we call CinQConnections. Through CinQConnections, CINQCARE Family Members get the year-round support they need, and at the level they need.

Here’s how it works:

Upon being assigned to Cinq Connect by your health plan, or being aligned by your Primary Care Provider, a Nurse Care Manager reviews your available health data and drafts a plan to meet your needs.

If more information is needed, we may ask you to complete a simple health risk assessment annually. Depending on your needs, over the course of the year, Family Members may receive:

Your Care Coordinator will help you when needed to:

  • Chose a Primary Care Provider:

    We offer a network of culturally appropriate primary care practitioners, who specialize in working with black and brown family members as well as those in geographically under-served areas.

  • Arrange specialty care, home health care, or other services including those covered under other benefits such as Medicaid.
  • Address Social Determinations of Health such as housing, food or financial insecurity
  • Get a test or procedure.
  • Extend short term a prescription
  • Pick up or arrange home delivery of medications.
  • Arrange Transportation

For Family Members on multiple medications or who need help managing their medications (insert link to existing program description on website)

For Family Members with gaps in care or who have not been in to see their PCP as often as much as needed we offer a house call in the home or via telehealth. Learn more at (https://cinq.care/care-at-home-annual-visit-program/)  

Post Emergency Room, Hospital or Skilled Nursing Facility Discharge, a member of your Care Team will follow-up to ensure you receive aftercare within 7 days and/or a medication reconciliation post-discharge.

Our Care Coordination Program supports other services you may receive from your health plan and supplements but does not replace the care provided by your primary care providers. Enrollment is free with co-pays waived for services rendered through our Care at Home Practice.  

Members may opt out of enrollment at any time. 

You may be eligible for gift cards or other incentives for meeting health goals.

Annual Assessment Visit
Program

Our Care at Home Annual Assessment Visit Program ensures that you receive comprehensive care through an annual health check, conducted in the comfort and safety of your home. There is no charge for this program, and it is particularly beneficial for those who have been unable to visit their doctor’s office regularly or as needed. The results of this assessment will be sent to you and those you designate along with a care plan to help you stay healthy and take care of your social needs throughout the year.

Concierge Program

At CINQCARE, we understand the importance of having a caring support system in place. We work alongside your caregiver, whether it’s a member of your family or another helper, to provide you with ongoing care to help you manage your chronic illnesses such as heart disease, asthma, or diabetes. At no charge, we provide 24×7 access to a provider, who works with you to monitor important daily changes in your conditions such as weight or sugar readings. These help us keep you safe at home and out of the hospital.

Clinical Pharmacy Service

Care at Home offers its Family Members a personalized telehealth service that connects them with expert clinical pharmacists in a secure, private and convenient way. Family Members will learn ways to make their medication regimen more efficient and effective, save money and avoid side effects and potential medication interactions.

Working with your provider, we will help you manage your medications, make recommendations and help you stay healthy. An appointment with a clinical pharmacist can be made by calling 716-354-8917 for by going online at https://cinqcare.as.me.
